Rising Sixth Graders

Have a fifth grader going to middle school next year?

Here is information on the immunizations required for middle school students:

  • Tdap (Booster) - for all rising 7th graders and new enrolling eight-12th graders (tetanus, diphtheria and pertussis).
  • Meningococcal - (Effective July 1, 2021) The first dose is administered prior to 7th grade. Rising 12th graders will be required to have dose prior to school entry.
  • Human Papillomavirus Vaccine (HPV) - Effective, October 1, 2008, a complete series of 3 doses of HPV vaccine is required for females and males. The first dose shall be administered before the child enters the 6th grade. After reviewing educational materials approved by the Board of Health, the parent or guardian, at their discretion, may elect for the student not to receive the HPV vaccine.
